Party Pre-Booking Form

By completing this booking, I agree to the following:

Booking & Payment

• A non-refundable deposit is required to reserve my party date and time.

• My booking is not confirmed until the deposit is received.

• The remaining balance is due 7 days before the event.

• Failure to pay the balance on time may result in cancellation.

Guest Count

• My party package includes a set number of children.

• I will provide a final guest count at least 5 days prior to the party.

• I understand I will be charged for any additional guests.

• No refunds will be issued for unused spots.

Timing

• I may arrive 15 minutes early for setup.

• My party will end at the scheduled time.

• Late arrivals do not extend the party duration.

Supervision & Safety

• I am responsible for supervising all children at my party.

• All participating children must have a signed waiver on file.

Food & Decorations

• Outside food is permitted for parties only (including cake).

• No nuts, confetti, glitter, or wall-damaging decorations are allowed.

• No face painting.

• I am responsible for informing guests of any allergy policies.

Deposit & Cancellation Policy

• My deposit is non-refundable.

• If I cancel 14+ days in advance, I may transfer my deposit to a new date (subject to availability).

• Cancellations within 7 days of the event are non-refundable and non-transferable.

• One reschedule is allowed if requested at least 7 days prior, and must be used within 60 days.

No-Shows & Emergencies

• No-shows or same-day cancellations forfeit all payments.

• In case of illness or emergency, PlayPatch may offer a one-time reschedule at its discretion.

Weather Policy

• If PlayPatch must close due to weather or emergency, I will receive a full credit or reschedule option.

Damages

• I agree to be responsible for any damages caused by my party guests.
